Application Procedure
Step 1. Online Application Form
Access the Online Application Form here:
https://bit.ly/LSGHApplicationFormSY24-25.
Step 2. Payment of Processing Fee
A fee of Php 700.00 will be required as an admissions processing fee.
Step 3. Sending of Requirements via Email
Digital copies of the requirements may be sent after Step 2 to admissions@lsgh.edu.ph.

Step 4. Assessment Interview / Placement Test
Once the submission of required documents has been completed and after they are evaluated and approved, an assessment interview/placement test will be scheduled.
- Preschool (Pre-Nursery, Nursery, Kinder) to Grade 3 – Assessment Interview
- Grades 4 to 11 – Placement Test (paper and pen exam) and Assessment Interview
Step 5. Admissions Result
Results will be sent via email after seven (7) working days.

1. Signed Page 2 of Online Application Form
- After the online application form is submitted, an auto-response of the online form will be sent to the email address indicated.
- Please print Page 2 and affix your signature on the space indicated.
- Scan or take a clear photo of the page and save the file for uploading.
2. PSA-issued Birth Certificate
- Prepare a scanned copy of the student’s PSA-issued birth certificate.Please note that a hard copy of the birth certificate will be required for submission to the Admissions Office upon enrollment.

3. Two (2) Report Cards
Prepare scanned copies of the following:
- Final Report Card for SY 2022-2023 (previous grade level)
- Latest Available Report Card for SY 2023-2024 (current grade level)

Additional for Gr. 11 applicants: Grade 7 and 8 Report Cards

Notes:
- Excellent grades, satisfactory conduct and regular attendance are highly recommended.
- For Kinder and Grade 1 applicants, a Certificate of Enrollment or Progress Report will suffice.
4. Two (2) Recommendations from current school
For Gr. 2 to 11 only; one (1) Recommendation for Gr. 1
- 1 Recommendation from either Principal OR Guidance Counselor
- 1 Recommendation from either Homeroom Adviser OR Subject Teacher
Note: Form will be emailed by the Admissions Office to parent/guardian; to be forwarded to selected personnel from the current school.
5. For Foreign Student-Applicants:
Prepare scanned copies of the following:
- Alien Certificate of Registration (ACR)
- Student’s passport

Please be advised that the school strictly implements a cut-off age for incoming Pre-Nursery, Nursery, Kinder and Grade 1 applicants in line with DepEd Order no. 47, series of 2016.
